Silent Hill: Townfall has five endings to unlock, including two that are only available in New Game Plus.

The first playthrough endings are tied to overall play style rather than specific choices. Bloodthirstiness, exploration and lore discovery all affect the outcome.

The easiest ending is the worst outcome and is based on kills, self-revives and ignoring collectibles. It can also be used to help unlock the Reject The Help achievement/trophy by avoiding manual saves on a first playthrough in normal mode. This ending unlocks Just Making Things Worse.

A better ending requires a high score in an opaque system that is not shown in the game. The safest approach is to avoid kills while playing as Simon and collect as many Discoveries as possible. Reviving Simon with Filled IV Bags or the Enhanced IV Bag harms the score, while other healing items can be used freely. Zoe only needs to avoid damage during Chapter 7: The Patient, and neither her kills nor Richard's count negatively. This ending unlocks For St. Amelia.

The normal story outcome is the hardest to unlock because it needs a narrow score window. Since kills and self-revives reduce points, a manual save near the end can be used to allow repeated kills and deaths before finishing the game. This ending unlocks Here To Stay.

The first New Game Plus ending requires three coins and three Guide Dogs for the Blind banks.
  • The Bronze Coin is on shelving in the rear room of the Video Star store in south-west Shorefront District – South.
  • The Guide Dogs for the Blind bank is near the Pharmacy in north-east Shorefront District – South.
  • The Silver Coin is on shelving in a yard on the right of the intersection in Clara Woods.
  • The Guide Dogs for the Blind bank is in the laundry behind the post office in central Clara Woods Ground Level, after getting bolt cutters.
  • The Gold Coin is on top of a control console in Room 793, and the door to the valve must be unlocked at the same time using the panel.
  • The Guide Dogs for the Blind bank is directly in the path when leaving this section.
The end boss still has to be defeated to reach this ending, which unlocks Let's Talk This Out.

The second New Game Plus ending requires three Strange Books and a Strange Access Card.
  • Strange Book 1 is in the bedroom of Zoe's House, under the nightstand, with a hairdryer nearby.
  • After accepting Richard's gift, enter the hotel in Shorefront District – South and go upstairs to the symbol combination-locked door. Use the Strange Postcards' hints to unlock it, then collect the Strange Access Card from the nightstand.
  • Strange Book 2 is on the middle level of a bookshelf in a shop at the very south-west corner of Clara Woods Ground Level.
  • Strange Book 3 is on a table in Room 621, and the door to the valve must be unlocked at the same time using the panel.
Near the end, at the door that triggers the Simon cutscene with his IV, use the Strange Access Card on the keypad on the left wall. Going through the door then starts the Sci-fi ending, skips the boss fight and the epilogue, and unlocks Sci-fi-lent Hill.
